Note: You must use this method for any one of the reasons listed below:

• The applicant is currently under the age of 16 years old

*It doesn’t matter if the applicant has had a passport before now, only that they are under the age of 16 now. It is required to submit the current passport along with the application form; however, it must be submitted along with a certified copy of a State or County issued long form birth certificate to prove who Parents are. The existing passport for the Minor doesn’t state who the Parents are.
